How LDM® Triple Works
LDM® Triple delivers very high frequency ultrasound, and it delivers more than one frequency at a time. That second point is what separates it from the ultrasound already familiar in aesthetics.
Four frequencies, four working depths
Each frequency works at a different level. 19 MHz works in the upper 1 to 2 mm of the skin, 10 MHz to around 3 mm, at the boundary between the dermis and the fat beneath it, 3 MHz through the full subcutaneous fat layer to around 20 mm, and 1 MHz deeper again, into fascia and muscle. LDM® Triple is not a surface treatment. The deeper frequencies are a substantive part of what it does, and the frequency mix is what sets the working level for a given indication.
Three frequencies at once, one combined response
In Triple mode the device alternates between three frequencies, switching every 1 to 10 milliseconds. Skin cells and tissue structures need longer than 100 milliseconds to respond to a stimulus, so they cannot answer each frequency separately. They respond to all three as a single combined stimulus, and that combined response is different from the response to any one of the frequencies on its own.
Two biological targets
Two targets sit behind the range of indications: Caveolin-1, a protein governing the pathways behind inflammation, collagen turnover and repair, and dermal white adipose tissue, the fat layer lying immediately under the dermis.
Caveolin-1 stabilises caveolae, the small pits in a cell membrane where the cell concentrates its receptors. Its level is not uniformly wrong in disease: it is raised in aged skin and at the edges of chronic wounds, and reduced in psoriatic epidermis. That direction-specific picture is established in the independent literature, not a manufacturer claim, and is set out in Takamura and Yamaguchi, Frontiers in Immunology, 2022.
What is proposed, rather than demonstrated, is that multifrequency ultrasound moves Caveolin-1 back toward normal levels in either direction, through mechanical stress on the cell membrane and the cytoskeleton. That argument is the manufacturer’s, set out by Ilja Kruglikov in the Journal of Cosmetic Dermatology in 2025. No published study has yet measured Caveolin-1 in human skin after LDM® treatment, so the mechanism is the manufacturer’s account of why the published outcomes occur, and the outcomes are what the clinical evidence records.
Dermal white adipose tissue is distinct from the deeper subcutaneous fat. It thickens as the dermis thins with age, it cycles with the hair follicle, and it takes an active part in inflammation and scarring rather than sitting passively beneath the skin. It is also exactly where the 10 MHz and 19 MHz frequencies produce their steepest effect.
What this means for ageing skin
The anti-ageing effect is attributed principally to realignment of the collagen fibres already present, and to increased bonding between them, rather than to the production of new collagen. New collagen is the third mechanism in order of importance, not the first. This is worth knowing when a patient asks how it compares with a device sold on collagen stimulation: the two are doing different things.

No consumables, so the margin holds
The only consumable is standard ultrasound gel. There are no tips, cartridges, single use handpieces or per session licence fees, so the tenth session of a course costs the clinic what the first one did. Nothing has to be reordered to keep treating and nothing expires on the shelf, so a busy month does not arrive with a supply bill behind it. It also means a course can be priced as a course without a consumable cost sitting under every session in it.
A course, not a single appointment
LDM® Triple protocols run as a series. The standard cadence is twice weekly with about three days between sessions, and treatment frequency is what makes results land, so the course is what the clinic sells and what the patient commits to. Improvement continues after the final session, which makes the review at three to four months a natural point to plan the next block of work rather than a courtesy call.
It earns its room
A session takes 20 to 30 minutes. There is no anaesthetic, no thermal injury, no downtime and no aftercare restriction, so LDM® Triple does not need an appointment of its own to be useful. It works before an energy based or needle based treatment to prepare the skin, and afterwards at lower settings to settle it, inside a visit the patient has already booked. In a survey of 40 dermatological and aesthetic clinics working with the technology, each clinic used an average of nine of the nineteen listed indications often or very often, and around thirteen of them in all.
It sits alongside the devices you already own
LDM® Triple is not a lifting or resurfacing device and does not compete for those appointments. Because it is non-thermal, it treats the patients those devices have to turn away: inflamed, reactive and compromised skin, acne, eczema and rosacea, and anyone who cannot accept downtime. It suits every season and every skin type, which matters in a market where months of strong sun keep laser and peel patients out of the chair.
Professional equipment, supplied through licensed channels
The Emirates Drug Establishment classifies LDM® Triple as a medical device, restricted to use by licensed healthcare professionals and supplied only to licensed healthcare facilities. It is not supplied to consumers in the UAE. For a clinic that means the treatment stays inside the clinic, and a patient who wants it has to book with someone who has one.
What comes with the device
Every LDM® Triple placed in the UAE comes with installation, on site clinical training, a two year warranty and help building it into the treatment menu, together with the LDM Launch Program run with LTF Digital: a three month marketing programme covering paid campaigns, a videoshoot at the clinic, a dedicated landing page, lead nurturing over WhatsApp and email, and monthly review calls. Clinics that want to carry on with it after the three months can do so directly.

LDM® Triple is made by Wellcomet GmbH in Germany, and the research behind it is led by Dr Ilja Kruglikov, whose published work on skin biology underpins the technology. LDM® stands for Local Dynamic Micromassage, which is Wellcomet’s own name for the method. Anglo Arab Trading FZCO supplies LDM® Triple to clinics across the UAE and the Gulf.
The technology is used in several settings: medical aesthetic clinics, dermatology practices, plastic surgery aftercare, and regenerative and functional medicine. What those settings share is a need to work on inflamed or compromised skin without putting the patient out of action, which matters here, where heat and sun exposure keep barrier problems in circulation all year.

Who is allowed to operate LDM® Triple in the UAE?
The Emirates Drug Establishment classifies it as a medical device restricted to use by licensed healthcare professionals, and supplied only to EDE, DOH or DHA licensed healthcare facilities. It is not supplied to consumers in the UAE.
What does it cost to run, session by session?
Standard ultrasound gel and nothing else. No tips, no cartridges, no single use handpieces, no per session licence fee. The tenth session of a course costs the clinic what the first one did.
How long is a session, and does it need its own appointment?
20 to 30 minutes, and no. It can be a treatment in its own right or an addition to a visit already booked, before another treatment to prepare the skin and afterwards at lower settings to settle it.
Does it work alongside the devices we already run?
Yes, and the sequence matters. Higher parameters before an energy based or needle based treatment, lower parameters afterwards. Wellcomet supplies orientation settings for laser, IPL and RF microneedling, and these are set per clinic during training rather than pre-programmed.
Are there patients or timings we should plan around?
Yes, and they are covered in training. LDM® Triple is not used in an area treated with hyaluronic acid filler within the previous three months, although it can be used immediately before filler in the same appointment. Treatment is deferred over active skin infection, anything requiring systemic therapy is referred, metal implants are assessed individually, and the applicator is never applied directly over the eyeball. Pregnancy is not a contraindication and the manufacturer confirms this. Confirm the device is listed on the clinic’s liability insurance before first use.
